Job Description:

Signify Health, part of CVS Health, is seeking a Part Time Physician to provide In-Home Health Evaluations, engaging with people in their homes and helping bridge gaps in care. This includes a medication and medical history review, a physical evaluation, and potentially additional diagnostic tests, using an intuitive clinical workflow.

Job Responsibility:

Conduct virtual visits as needed
provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
be available and flexible to travel within licensed states, up to 100% of working time
position requires overnight hotel stays, car rentals, and/or flights, as business needs dictate

Requirements:

Active, unrestricted license(s) in coverage area(s)
multi-state licenses (preferred)
able to cover the following primary locations: coverage in New Hampshire and Vermont (and other licensed states as needed)
