Job Description:

This role provides oversight of stroke quality metrics, regulatory readiness, and performance improvement initiatives across the hospital and affiliated facilities. You will work within the Neurosciences service line, with primary office location on the unit, supporting system-wide stroke program operations.

Job Responsibility:

  • Monitor, analyze, and report key stroke quality metrics
  • Collaboratively develops and implements disease management protocols/guidelines
  • Identifies, develops, and participates in hospital and community programs related to stroke
  • Develops or revises stroke written plans of care and guidelines
  • Communicates guidelines and plans of care to the multidisciplinary team
  • In collaboration with the stroke Physician Liaison and Administrator for Quality and Clinical Integration, establishes clinical benchmarks and quality outcome measures
  • Lead multidisciplinary performance improvement initiatives to optimize patient outcomes
  • Ensure ongoing regulatory readiness and compliance with stroke certification standards
  • Coordinate stroke committee meetings, case reviews, and action plans
  • Provide education and performance feedback to clinical teams across departments and affiliated facilities
  • Collaborate with Emergency Department, ICU, Radiology, Neurology, Rehabilitation, and system partners to ensure continuity of care

Requirements:

  • Registered Nurse license issued by the state in which the team member practices
  • Bachelor's Degree in Nursing
