Above All Talent Solutions is partnering with a rapidly growing therapy provider that delivers innovative rehab programs to senior living communities. We are seeking a dedicated Speech Language Pathologist (may consider driven CFYs) to join this progressive team and provide person‑centered services to adult and geriatric residents in senior living and post‑acute settings.
Job Responsibility:
Evaluate residents for speech, language, cognitive‑communication, voice, fluency, and swallowing disorders, and develop individualized, goal‑oriented plans of care
Treat adult and geriatric communication disorders, including speech sound production, language and aphasia, literacy‑related issues, social communication/pragmatics, voice, and fluency
Assess and treat cognitive‑communication deficits affecting memory, attention, problem solving, organization, and safety awareness as they relate to functional daily activities
Evaluate and treat feeding and swallowing (dysphagia) disorders, recommending appropriate diet textures, liquid consistencies, and compensatory strategies to support safe nutrition and hydration
Collaborate with the interdisciplinary team (nursing, rehab, physicians, dietitians, and wellness staff) to integrate speech‑language and swallowing goals into overall resident care plans
Educate residents, families, and staff on communication strategies, cognitive‑communication supports, safe swallowing techniques, and use of adaptive equipment or technology
Document evaluations, treatment sessions, progress notes, and discharge summaries accurately and in a timely manner, in accordance with regulatory, professional, and payer requirements
Participate in care conferences and quality initiatives to support improved outcomes, reduced rehospitalizations, and enhanced quality of life for residents
Requirements:
Current, unrestricted Speech Language Pathologist license in the state of practice (or eligibility prior to start)
Certificate of Clinical Competence in Speech‑Language Pathology (CCC‑SLP) preferred, if applicable by state and employer requirements
Prior experience in senior living, skilled nursing, post‑acute rehab, or long‑term care strongly preferred
Strong interest and competence in adult and geriatric communication and swallowing disorders
Excellent communication, documentation, and collaboration skills, with a resident‑centered, outcomes‑focused approach
