The Hospice Social Worker is assigned a group of patients to oversee social work care for both patients and families, and provides patients and family members with the education and advice needed to cope with issues which arise from various emotional or physical problems as outlined by the Texas State Board of Social Worker Examiners scope of practice. Coordination includes a variety of services including crisis intervention, bereavement, client advocacy, referrals, education, discharge planning and implementation, counseling and psychosocial assessments.
Job Responsibility:
Provides patients and family members with education and advice to cope with emotional/physical problems
Develops written plan of care for each patient
Conducts patient assessments as required by Medicare
Coordinates patient respite stays
Serves as liaison with local community resources
Assists with admissions, discharges, transferring
Participates in Hospice Annual Memorial service
Coordinates with Texas Adult Protective Services when needed
Requirements:
Masters' degree
1 Year of Experience
Lic Masters Social Worker (LMSW) or Lic Clinical Social Worker (LCSW)
Demonstrates critical communication skills
Knowledge of principles and methods to restore/enhance functioning
Able to develop therapeutic relationships
Function independently in the community
Able to drive own vehicle and travel within service area
Functions as a team member
