Job Description:

At HammondCare Cardiff, you will leverage your skills and expertise, to support an excellent Care team to improve the quality of life for people with dementia. HammondCare’s unique model of care, places our residents needs as your number one priority, creating a homelike environment, you will be supporting the delivery of person-centred, relationship focused care.

Job Responsibility:

Advising and mentoring and leading a team of Specialised Dementia Carers to support the needs of our residents
Supported by specialist dementia trainers to become specialist in your field
Partnering with another senior RN on each shift, providing mutual support and collaboration throughout the night
Providing clinical assessment and input into how best to support the changing needs of our residents in all aspects of their lives
Consulting and mentoring in case management, supporting clinical decision making and professional oversight as a registered nurse
Implement and support resident comfort focus
Build relationships with residents, in alignment with our unique Model of Care, prioritising and respecting each person as an individual

Requirements:

Must be available to work Nights week 1 – Monday to Wednesday - 11pm - 7am & week 2 - Monday to Thursday 11pm - 7am, 52.50 hrs FN
Patience, empathy and a passion for improving quality of life for older people with dementia
Value team work, but also the ability to work independently and show initiative
Have current AHPRA registration with ideally 2-3 year of experience in an Aged Care setting
