Job Responsibility:

  • Monitoring and Documenting medication self administration during clients’ stay
  • Monitoring client physical medical issues and working with Mental Health Workers to arrange for acute medical care for clients when needed
  • Facilitating health and wellness groups and activities
  • Delivering treatment interventions that are strength based and solution-focused
  • Encouraging consumer completed risk management/safety plans and Wellness Recovery Action Plans (WRAP)
  • Coordinating with clients’ outpatient and CCS care providers
  • Completing documentation per shift as assigned
  • Participating in ongoing required training

Requirements:

  • Must demonstrate the following characteristics: reliable, responsible, team player, ability to grow and learn in the position, detail-oriented, organized, flexible, capacity to provide support and quality care to improve the physical and mental health of individuals served
  • Must be an independent problem-solver, able to work with minimal supervision when necessary
  • Graduate of an accredited Nursing program (Diploma/Associates/or Bachelor’s degree)
  • Registered Nurse (RN) must be licensed by the Massachusetts Board of Registration

Nice to have:

  • One-year experience in an acute mental health and/or dual diagnosis setting
  • Familiarity with Recovery oriented treatment for severe mental illness

  • Performs initial nursing evaluation, including behavioral health/psychiatric nursing needs
  • Observes and evaluates the physical and mental status of the patient for complications and medication side effects
  • Maintains awareness for indicators in patients' behavior that call for change in the treatment modality (crisis, suicidal ideation) as well as other medical signs and symptoms
  • Instructs patient/caregiver in cognitive and technical aspects of care, medication regimes, disease processes, safety interventions, ADL care, coping skills, symptom management and other interventions
  • Administers IM medications, performs lab draws and specimen collections
  • Acts as a case manager in coordinating care for patients as assigned
  • Completes documentation timely and accurately
  • Supervises LPNs and HHAs
  • Regularly communicate patient progress to the clinical manager, psychiatrist and care team
