• Engage with people in their homes
• Help bridge gaps in care
• Conduct virtual visits as needed
• Provide Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)
• Build trusted relationships to make people healthier

• Active, unrestricted license(s) in coverage area(s)

• Able to cover the following primary locations: Manhattan and surrounding area
