Job Description:

The role involves providing In-Home Health Evaluations, engaging with patients in their homes, reviewing medical history, performing physical evaluations, and facilitating diagnostic tests without prescribing medication or altering treatments. You will use tablets and follow an intuitive clinical workflow.

Job Responsibility:

Bring your heart into every visit, joining a national network of clinicians improving health outcomes
Visit people nationwide and provide connections to the right care for unique needs
Conduct virtual visits as needed
Provide all Signify Health services including Diagnostic & Preventive Services products

Requirements:

Active, unrestricted license(s) in coverage area(s)
Multi-state licenses (preferred)
Able to cover the following primary locations: Rochester and surrounding area
