Above All Talent Solutions is partnering with a rapidly growing therapy provider that delivers innovative rehab programs to Senior Living communities. This full-time Physical Therapy Assistant position may have potential to support nearby sister facilities, serving adult and geriatric residents in a post-acute settings. As a key member of an interdisciplinary rehab team, you will help residents improve mobility, strength, balance, and pain management to maximize safety, independence, and quality of life.
Job Responsibility:
Implement physical therapy treatment plans developed by the supervising Physical Therapist, including therapeutic exercises, gait training, and functional mobility activities
Instruct and assist patients with exercises, transfer techniques, balance and strengthening activities, and use of assistive devices (walkers, canes, wheelchairs, braces)
Monitor resident responses to therapy before, during, and after treatment, modify activities within the plan as appropriate, and promptly report changes in condition or concerns to the Physical Therapist and nursing team
Provide 1:1 or small-group therapy sessions focused on mobility, fall reduction, endurance, and functional tasks needed for safe aging in place within the senior living setting
Assist with collecting data for initial and ongoing assessments by interviewing residents, observing movement and function, and reviewing therapy histories and relevant test results as directed by the PT
Contribute input to the PT regarding progress toward goals, barriers to participation, and recommendations for updating the plan of care, equipment needs, or level of assistance
Participate in interdisciplinary meetings or care conferences to communicate therapy goals, progress, and discharge plans to nursing, wellness staff, and families as requested
Requirements:
Current, unrestricted Physical Therapy Assistant license in the state of practice (or eligibility prior to start)
Prior experience in skilled nursing, post-acute rehab, or long-term care strongly preferred
Excellent communication, documentation, and collaboration skills
