Job Description:

The Nurse Practitioner will provide value-based primary care to adults on Medicare in an outpatient setting, focusing on building meaningful patient relationships, promoting care coordination, and championing population health.

Job Responsibility:

Provision of exceptional primary care
Conduct office visits for routine and acute issues
Administer Annual Wellness Visits and health risk assessments
Care coordination with providers, specialists, testing facilities, and agencies
Population health leadership
Assist the care team with phone triage and outreach
Educate patients on health conditions, care plans, and treatments
Participate in Oak Street Health promotional activities
Conduct home visits as needed

Requirements:

Master of Science in Nursing
Doctor of Nursing Practice
NP Certificate Program graduate
National certification in at least one specialty
Active, non-probationary state Nurse Practitioner license
Active DEA license
US Work Authorization

Nice to have:

Experience in primary care: internal medicine, geriatrics, or family medicine
Understanding of HCC documentation, ICD-10 coding, and HRAs
Passion for teamwork and cross-functional collaboration
Interest in innovative, population health-focused models
Bilingual proficiency in applicable areas
