Job Description:

Signify Health, part of CVS Health®, seeks a Part Time Clinician to provide In-Home Health Evaluations. This involves conducting physical evaluations, answering health questions, reviewing medication history, and performing diagnostic tests as ordered by the person’s health plan.

Job Responsibility:

Conduct virtual visits as needed
Provide Signify Health services including Diagnostic & Preventive Services products
Help bridge gaps in care
Build trusted relationships to improve health outcomes
Travel locally and within licensed states as needed

Requirements:

Active, unrestricted license(s) in coverage area(s)
Board certification

Nice to have:

Multi-state licenses
