Job Description:

This role as a Nurse Practitioner at Oak Street Health focuses on providing primary care to adults on Medicare, building meaningful patient relationships, and collaborating with a team of healthcare professionals in an innovative care model driven by population health.

Job Responsibility:

Provision of exceptional primary care
Conduct office visits for routine and acute issues
Administer Annual wellness visits and health risk assessments
Care coordination with other providers, specialists, testing facilities, and agencies
Population health leadership in coordination with the Care Team
Assisting the care team with phone triage and outreach
Educating patients on their health conditions and care plans
Participating in Oak Street Health promotional activities
Conducting home visits as needed

Requirements:

Master of Science in Nursing
Doctor of Nursing Practice
NP Certificate Program graduate
National certification in Family Nurse Practitioner or other listed specialties
Active, non-probationary state Nurse Practitioner license
Active DEA license
US Work Authorization

Nice to have:

Experience in primary care: internal medicine, geriatrics, or family medicine
Understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments
Passion for teamwork and collaboration
Desire to be part of an innovative model focused on empirically-guided population health
