This is a one-year paid fellowship program in Adult Primary Care for new graduate Nurse Practitioners (NP), aimed at training them within Oak Street Health’s value-based care model. The fellowship prepares NPs to become competent clinicians for older adults, building their own patient panels, engaging with preceptors, and learning didactic content to enhance their practice.

Requirements:
Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their training program between December 2023 and May 2025
Currently residing in or willing to relocate to program location for at least the next two years
Have not yet worked as an NP
Minimum of one year experience, within the last five years, working as an adult RN
Minimum GPA of 3.0 in NP program
Currently holds or will hold by 9/15/2025 an active, non-probationary state NP license
A genuine passion for adult primary care
