This is a 12-month paid fellowship program at Oak Street, providing necessary skills and learning to become a confident clinician for older adults. It focuses on value-based care and includes building a patient panel, learning from preceptors, and engaging in specialized training. Participants must commit to working at Oak Street Health for at least a year post-fellowship.
Job Responsibility:
Spend time building their own patient panel
learn from a multitude of preceptors
engage in didactic content that aims to build on, not repeat, graduate training
improve the health of communities across the country
drive clinical excellence
provide personalized primary care for older adults
Requirements:
Family, adult, or adult-gerontology primary care nurse practitioners who graduated or will graduate from their training program between December 2023 and May 2025
currently residing in or willing to relocate to program location for at least the next two years
have not yet worked as an NP
minimum of one year experience, within the last five years, working as an adult RN
minimum GPA of 3.0 in NP program
currently holds or will hold by 9/15/2025 an active, non-probationary state NP license
a genuine passion for adult primary care
a supportive attitude toward our patient population of older adults
embrace teamwork and the opportunity to collaborate with brilliant colleagues

Spanish language skills are strongly preferred, depending on clinic location
