We are looking for enthusiastic and motivated keyworker and can offer a range of exciting opportunities to work across the community services and within our adult teams in developing the our service as outlined by NHS England and Navigo.
Job Responsibility:
To supervise and coordinate the clinical care of a defined caseload.
Assess, plan, implement and evaluate care to provide appropriate interventions to meeting individual need.
To assist in providing professional leadership, guidance and advice to junior team members and other members of primary and specialist services.
To maintain and enhance close professional working relationships with referring agencies and colleagues from other disciplines and the voluntary sector, offering professional guidance and advice in relation to Mental Health issues.
To be a member of the integrated Primary Care Network team CMHT, coordinating care across all disciplines and multi agency boundaries.
Manage a caseload utilising patient centred assessment, planning and implementation of care plans.
Evaluate care plans and outcomes including active participation in regular clinical audit.
Liaise with other members of the multi-disciplinary team thereby maintaining and enhancing close professional working relationships.
Participate in regular clinical supervision in accordance with the Navigo Policy for both clinical and managerial aspects of their work.
To record and maintain all professional, clinical information of patient care needs in a confidential manner.
To work within the guidelines for Professional Practice / Navigo Policy.
Carry out the requirements of the Mental Health Act 1983, Mental Capacity Act 2005 and NHS & Community Care Act 2014 as required.
Adhere to local policy regarding storage, carriage and administration (nurses only) of prescribed medication.
Keep abreast of professional development and updating requirements.
To actively link in with the Primary Healthcare Team, the Multi-Disciplinary Team and Care Programme Approach reviews.
To work with individuals and families at a variety of venues including clients houses, hospital bases, GP surgeries etc.
Participate in the service duty system as required.
To act as Key Worker
Acts as an Approved Social Worker/Approved Mental Health Practitioner if qualified to do so.
Undertake specific related tasks commensurate with professional background
Supervise and assess students and support students of other disciplines as required, providing opportunities for learner objectives to be addressed.
To participate in educational development as required, enhancing mental health skills and knowledge with reference to leaner needs.
To act at all times as a professional role model for allocated learners and colleagues.
To manage own time effectively and support the management with leadership elements of the team.
Submit accurate information as required in keeping with the information system and additional departmental needs.
To act as a supervisor in providing clinical supervision to junior colleagues within the team.
To participate in the annual Performance and Development Review process.
To act in accordance with Health and Safety at Work Act 1974 the Fire Precautions Act 1974 and the COSHH.
To maintain a personal portfolio for purposes of professional registration.
Other work and general duties as delegated within the overall purview of the post.
Requirements:
Registered professional qualification (Registered Nurse/OT/SW)
Post basic education/training in Mental Health.
Ability to write clear concise records and reports.
Good communication, organisational and administration skills.
Supervision & Leadership skills
Knowledge of Community Care Act, Mental Health Act and related policies. (including safe working)
IT skills.
To work flexibly to meet the needs of the patient group.
Undergo future training
Dress appropriate to occasion
Able to drive or otherwise transport self conveniently across the district/area in accordance with the need of the task
Previous experience in a community mental health environment.
Minimum of 2 years experience at Band 5 level in mental health.
The post holder must have a current UK driving licence and access to a vehicle.
Applicants must have current UK professional registration.

Previous supervisory experience.
Additional direct experience or academic study relating to mental health issues.
