Job Description:

Job Responsibility:

Help people leave hospital and transition back home
Visit Service Users in their own homes to provide care and support as defined in the Care Plan
Provide personal care, administer medication, monitor vital signs, assist with exercise and domestic tasks
Liaise with medical professionals such as district nurses and pharmacies
Rehabilitate and maximise potential
Support and enable Service Users in activities to promote independent living
Monitor well-being

Requirements:

Access to own vehicle
Right to work in the UK (no sponsorship provided)
NVQ level 2 or above in Health and Social Care (for higher pay rate)
