Certified Healthcare Access Associate (CHAA) successfully completed within 18 months of employment. Certification must be maintained while employed.
One to two years of direct clinical experience in Emergency Department/Critical Care Unit (such as Critical Care Tech EMT-B etc.), or one to two years of full time direct administrative healthcare experience (such as Nursing Unit Secretary or Patient Access Representative) or one to two years Transfer Center experience.
Working knowledge of medical terminology, excellent computer skills, customer service, ability to multi-task efficiently and effectively.

Job Responsibility

Coordinates all processes, support, communication and documentation involved in transferring patients from one facility to another facility, as well as support physician consults.
Transfer Coordinators also required to recognize and uphold all regulatory requirements, while simultaneously updating key stakeholders of timelines.
Accurately enters all required data into EPIC database.
Coordinates with Flight for Life for patient transports.

Job Responsibility

Providing excellent medical care and treatment for patients in their home or wherever they request with the supervision/oversight of an attending Physician or Physician Assistant
Assessing and treating Covid-related cases which includes obtaining clinical samples to test for the virus, performing point of care testing , obtaining blood for antibody blood tests, providing general symptom management and recommendations
Performing medical procedures that include obtaining IV access, phlebotomy, administering vaccines, performing POC rapid tests, administering IV medications and fluids, and performing EKGs
Documenting patient information in the electronic health record, Athena Health and in our CRM Salesforce
Responsibility for maintaining and tracking medication inventory
Coordinating additional care with our After Care, and house calls teams or with one of our Medical Partners
