Job Responsibility:
conducting structured medication reviews
running long-term condition clinics
providing enhanced clinical supervision across PCNs
offering expert prescribing support and patient-focused interventions
collaborating with GPs and the wider MDT to optimise medicines use
Requirements:
GPhC registration as a Pharmacist
experience in primary care, ideally in a GP surgery setting
proven expertise in structured medication reviews & long-term condition management
independent prescribing qualification (desirable but not essential)
strong communication and decision-making skills
