Job Description:

The Clinical Partner works within HammondCare’s Mission in Action to deliver high-quality, person-centered clinical care that supports clients to live well in their own home. The role provides evidence-based, best practice clinical care holistically, to individuals, ensuring their unique goals, needs and preferences are understood and addressed.

Job Responsibility:

Assessing clinical care needs
Developing and reviewing care plans
Delivering excellent clinical care in partnership with clients and key stakeholders
Working collaboratively with the care management team to ensure the best outcomes for clients in the community setting
Providing guidance to care staff to ensure the safe and continuous delivery of high-quality clinical care

Requirements:

Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action
Nursing qualification and current registration with AHPRA as a Registered Nurse (Division 1, General)
Passion for and previous experience in client-focused service delivery in aged and community/home care settings
Knowledge of the Aged and Home Care Reforms and Aged Care Quality Standards
Excellent communication and collaboration skills, with a commitment to advocacy and person-centred care
Ability to work effectively in a multidisciplinary team and independently across multiple sites
A current driver’s license and the ability to travel between HammondCare’s sites and services
Excellent administrative and computer skills, including digital health records and care coordination tools
Commitment to professional development
Empathy, patience, and a passion for delivering high-quality, person-centred care

Nice to have:

Nursing experience in aged care and community care
Ability to identify and implement innovations in care delivery and service improvement
