Job Description:

The Center Medical Director will lead the clinical team at Oak Street Health centers, mentoring colleagues while practicing primary care and implementing organizational strategies to ensure high-quality care for patients. The role involves 80% clinical work and 20% administrative work and requires clinical leadership experience, board certification, and a commitment to older adults and low-income patients.

Job Responsibility:

Work with center leadership to achieve organizational objectives
Drive OSH strategic initiatives
Implement regional priorities
Cascade organizational information and escalate concerns
Coordinate and lead onboarding of new providers
Mentor and coach providers
Ensure care model implementation
Educate providers on value-based risk model
Assess root causes for unmet metrics
Conduct performance reviews and develop provider goals
Celebrate center successes
Promote positive culture

Requirements:

M.D., D.O., A.P.N or P.A. graduates
Board certification in Internal Medicine or Family Medicine for M.D. or D.O.
Board certification in Family, Adult, or Gerontology for A.P.N.
Experience in clinical leadership roles
Experience using a metrics-driven approach
Commitment to older adults and low-income patients
Comfort with ambiguity
Problem-solving skills
Ability to remain calm
5+ years of experience in outpatient practice preferred
Fellowship training in Geriatrics or other professional degrees (e.g., M.B.A., J.D., M.P.H.) welcomed but not required
US work authorization
