The Center Medical Director role at Oak Street Health involves leadership of the clinical team to provide the best patient care, achieve strategic organizational objectives, and mentor providers. The CMD practices primary care alongside colleagues and has both clinical and administrative duties. Responsibilities include coordination of medical care, care model implementation, and fostering a positive culture while supporting care team development. The role focuses on thoughtful primary care practices and embodies the Oak Street Health mission to improve healthcare for older adults on Medicare.
Job Responsibility:
Support of Organizational Strategy including collaboration with Practice Manager and Outreach Manager/Director
Drive implementation of OSH strategic initiatives
Cascade organizational information and escalate care team concerns
Help coordinate and lead the onboarding of new providers at the center
Model excellent medical care and care model success
Support care model implementation, admission prevention work, and clinical documentation workflows
Support consistent and evidence-based medical care
Educate and provide ongoing support for providers in implementing the value-based full risk model of care
Assess and address root causes for unmet metric goals
Conduct performance reviews and develop plans for providers' goals
Create and maintain a culture of coaching and development
Celebrate team successes and communicate opportunities.
Requirements:
M.D., D.O., or A.P.N or P.A. graduates
Board certification in Internal Medicine or Family Medicine required for M.D. or D.O., Board certification in Family, Adult, or Gerontology for A.P.N.
Experience in clinical leadership roles, leading and coaching providers to be the best they can be for their patients and their colleagues
Experience using a metrics-driven approach to the provision of medical care and/or quality projects
Commitment to older adults and to low-income patients
US work authorization
5+ years of experience in outpatient practice preferred.
Nice to have:
Comfort with ambiguity and a love for problem solving
Fellowship training in Geriatrics, other professional degrees (e.g., M.B.A., J.D., M.P.H.), and prior executive roles welcomed but not required.
