Case Manager - East Boston. Supporting families experiencing homelessness in one of the region's largest shelter and stabilization programs, this role plays a key part in helping parents and children work toward safety, stability, and long‑term independence.
Job Responsibility:
Provide intensive case management, assessment, counseling, and advocacy for families in shelter
Build goal‑oriented, supportive relationships with families from intake through stabilization
Collaborate with Housing Specialists, Behavioral Health Clinicians, and Family Partners
Support families in developing rehousing and stabilization plans within a trauma‑informed framework
Maintain accurate case files and documentation for internal and external reporting
Deliver crisis intervention in partnership with behavioral health and leadership teams
Serve as a positive role model and advocate for families' needs across systems
Requirements:
Bachelor's degree in Human or Social Services
2+ years of case management experience
Background working with vulnerable or high‑needs populations
