Hello Neighbor is seeking a Preferred Communities GAPS Case Aide/Manager to provide stabilization-focused, trauma-informed case management services to refugees and ORR-eligible individuals in the local area. The Preferred Communities GAPS Case Aide/Manager supports individuals and families who require some additional case management and navigation support following their initial resettlement period. Services focus on addressing barriers to stability and self-sufficiency, including medical access, benefits navigation, transportation planning, and connection to community resources. This role emphasizes short-term engagement, systems navigation, and coordination of services rather than intensive or clinical case management. The GAPS Case Aide/ Manager works collaboratively with internal Hello Neighbor programs and external partners to ensure continuity of care, appropriate referrals, and non-duplication of services, using a trauma-informed and culturally responsive approach.
Job Responsibility:
Conduct intakes, assessments, and develop individualized stabilization and self-sufficiency plans for clients in accordance with Preferred Communities GAPS guidelines
Provide short-term, light-touch case management and navigation support based on identified client needs
Support clients in accessing healthcare, benefits, transportation, and other essential community resources
Initiate referrals to appropriate community-based services and ensure warm handoffs as needed
Advocate for the appropriate and consistent use of interpretation services across internal and external service providers
Provide extended cultural orientation related to systems navigation and community resources
Assist clients with transportation planning, including bus training and referrals to the Medical Assistance Transportation Program (MATP)
Support appointment coordination and follow-up to reduce barriers to service access
Participate in Office Hours on Wednesdays on an alternating schedule with other team members from the Family Health Services department
Collaborate with other Hello Neighbor programs to ensure continuity of care and avoid duplication of services
Adhere to Hello Neighbor’s confidentiality guidelines and ensure appropriate handling of sensitive information
Participate in regular supervision meetings and relevant staff meetings
Other duties as assigned within the scope of the position
Requirements:
Bachelor’s degree in social work, public health, human services, or a related field
Experience working with refugee, immigrant, or other vulnerable populations
case management or navigation experience preferred
Valid driver’s license, access to a personal vehicle, and proof of insurance
Personal auto insurance with a minimum $300,000 single combined limit of liability coverage
Alignment with and support of Hello Neighbor’s mission and core values
Bilingual or multilingual skills preferred, particularly in priority languages (e.g., Dari, Pashto, Arabic, Swahili, Rohingya, Burmese, Ukrainian, French)
Demonstrated ability to appropriately utilize interpretation services
Digital literacy and the ability to learn and navigate new software systems
Strong organizational skills and ability to manage multiple tasks effectively
Flexibility to work occasional evenings and weekends as needed
Must be currently authorized to work in the United States
Clearances required: Pennsylvania Child Abuse History Clearance, Pennsylvania State Police Criminal History Clearance, and Federal Bureau of Investigation (FBI) Criminal History Clearance, as applicable
Mandatory Reporter and/or PSEA training may be required depending on job responsibilities
A valid driver’s license, personal vehicle, proof of insurance, and motor vehicle record (MVR) may be required
