The role of Admissions Manager – Home Care plays a key role in advancing HammondCare’s Mission and Mission in Action by leading teams of Care Advisory Partners (CAP’s) to deliver high-quality, client-centred intake and onboarding experience. The role combines frontline assessment responsibilities with leadership and business development functions to drive growth, enhance client satisfaction and strengthen referrer and stakeholder networks. This role supports strategic growth by overseeing Home Care Package (HCP) and Commonwealth Home Support Program (CHSP) admissions, refining onboarding processes, and developing team capability.

Requirements:
Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action
Formal qualifications in a care-related discipline (minimum Certificate IV with additional micro-credentials
Diploma or Degree preferred)
Demonstrated experience conducting comprehensive assessments in a home care or aged care setting
Proven leadership experience with the ability to develop teams and manage performance, including experience leading and managing a team including remotely, as well as experience operating effectively in a multi-site environment
Sound knowledge of Aged Care Act
Home Care Packages Program Operational Guidelines
Aged Care Quality Standards and My Aged Care Portal and referral pathways
High-level communication and interpersonal skills with an ability to engage clients and stakeholders at all levels
Strong planning, time management and organisational skills
Analytical capability to use data to inform decisions and improve service delivery
Proficiency in client management systems and Microsoft Office
Current driver’s licence, reliable vehicle, and willingness to travel
